ENROLLMENT INFORMATION
New Student Enrollment:
1. Complete/Print: NEW STUDENT ENROLLMENT/REGISTRATION FORM
2. Collect the following items:
I. Proof of Residency (utility bill, mortgage payment, etc.)
II. Georgia Certificate of Immunizations (Form 3032 or Form 3231)
III. Georgia Certificate of Ear, Eye, Dental Examination and Nutrition (Form 3300)
IV. Certified copy of Birth Certificate
V. Copy of Social Security Card -or- Alternative Form Completion
VI. Parent Identification (drivers lisence, social security card, etc.)
3. Bring all items from #1 and #2 to CMS Front Office to Mrs. Kelly Faires, Registrar
**Ms. Sara Bahena is available for translation/language assistance**
4. Mrs. Faires will provide the School Information papers to be completed upon approval of all of the above, including:

CMS DAWG HOUSE STUDENT RESOURCE CENTER:
The Cedartown Middle School Dawg House is an in-house student resource center located in the CMS Media Center, including a clothing closet, toiletry cabinet, school supply closet, food pantry, and numerous private counseling and decompression areas for our CMS students and families. Resources are provided (over 85% by student, faculty, and community donations) to all students at any time. The entire area is maintained by Mrs. Laura Cox and her 5th Period Enrichment class as a service-oriented outreach program for our school and community.
